A clinical note template is a text-based template that is suitable for letters and other text-based documentation. Clinical note templates can include dynamic terms, tables and images.

A clinical tool is is a special type of clinical note that can include forms, questions and annotatable images that you or your patients can fill out. Answers to questions in clinical tools can be tallied and collated, as well as tracked over time using the Clinical Dashboard. Clinical tools can include patient intake or screening forms, questionnaires, diagnostic tools and annotatable images.

The most likely reason for this is because the clinical note is not linked to either a referral or an appointment, so Halaxy cannot detect what information to populate the dynamic term.
If you are editing a communication template, consider creating the the template as a reminder template instead because reminders are linked to appointments and referrals.
If you are editing a clinical note template, you must link the clinical note to an appointment or referral by following the steps below:

Open the clinical note.

In the title bar of the clinical note, click the
pencil icon.

In the pop-up, select the Appointment or Referral to link to this clinical note.

Click Save.
The dynamic terms should now display after you preview or publish the note. In addition, you will see the clinical note become a "child" of either the referral or the appointment you linked it to in the left-hand column (the note will be indented underneath the "parent" referral or appointment).
Note
To link a clinical note to both a referral AND an appointment, you must use the clinical note that is created automatically when you make an appointment in the calendar (the one named "Appointment" by default). You can rename it if you wish.
Yes. You can either upload your own annotatable picture or choose one from the Halaxy library. We currently have 60 annotatable images to choose from.

Click Settings > Clinical.

Click New Template at the top-right of the page.

On the left of the form, click New tools template.

Under Scoring, select Custom.

In the Questions/Fields section, click Add Another Question and enter a topic or heading as the Question / Title. The Add Template Question pop-up appears.

In the Answer Type drop-down menu, select Annotatable Drawing / Image.

Click Add File to add your own image, or click Preset to use one of the in-built Halaxy images.

Click Save.
