Create a patient form for professional contacts

By creating a patient form for your referring practitioners, you can allow them to submit both their own details and the referred patient's information, including the referral itself. After submitting, the referred patient is automatically added to your Halaxy patient list, reducing manual data entry.

You can also embed this form on an external website, such as your practice website, allowing referrers to complete and submit referrals, making the process even more accessible and convenient.

This workflow helps you streamline the referral process, save time on admin tasks, and ensure accuracy when capturing referrer and patient details. It's a simple and professional way to grow your practice by making it easier for referrers to send patients your way, wherever they’re submitting the form from.

Set up patient referral form

  1. Click Settings > General.

  2. Under the section Template Settings, next to Patient forms, click the Icon-Edit.svg pencil icon.

  3. In the top right, click Add Form, then select Professional Contact as your audience.

  4. Set up the form you want referring practitioners to complete. Make sure to enable the Patient Details section.

  5. Click Save.

You can now share the form link with referring practitioners or integrate it into your website. The form automatically includes a section for the professional contact's details.

Note

When a professional contact completes a form, a new patient or professional contact will be created and labelled Created via Form Link. If this created a duplicate of an existing professional contact or patient profile, simply merge the duplicate by opening the profile > Edit > Merge.

Caution

You cannot change the audience for an existing patient form.
