In Halaxy, practitioners can add a diagnosis or a problem record for a quicker review of the patient's information. This information can be added to the clinical sidebar within the clinical notes tab for quicker access.
Tip
You can easily add and track other types of health records - such as allergies or immunisations.

Open the patient's profile.

On the clinical notes tab, click on New Clinical Note.

Click Add Record and select Diagnosis.

On the note, fill in the following:

Problem / Diagnosis: Enter the problem or diagnosis and select it from the dropdown.

Onset (optional): Enter the month and the year of the approximate or actual date when the condition first occurred.

Body Site / Laterality (optional): Enter the location or region of the body that is affected.

Status (optional): Enter the status of the problem or diagnosis.

Verification status (optional): Enter the verification status of the problem or diagnosis.

Comments (optional): Enter any additional information related to the problem or diagnosis.

Click Save to save your progress and Publish to finalise the record.
You have successfully added a diagnosis or problem record for the patient.
When treating your patients' existing diagnosis or problem, it is crucial to provide updates to the information to monitor and keep track of their status.

Open the diagnosis or problem record.

At the top or bottom of the record, click on Add Update.

In the pop-up, enter the following that has an update:

Status: Select the clinical status of the problem or diagnosis.

Verification status: Select the verification status to support or decline the clinical status of the problem or diagnosis.

Comments: Enter any comments relevant to the problem or diagnosis.

Click Save.
You have successfully updated a diagnosis or problem record.
Note
Adding updates to a health record is only available when the record is published.

From the clinical sidebar, click on Settings.

In the Users list, click the group name (if you want to enable for your group sidebar) or your name (if you want to enable only for your own sidebar).

Under the Sidebar section, click Edit.

In the pop-up, turn on the toggle switch for Diagnoses. To reorder the tabs, click and drag the
up and down arrows.

Click Save.
The Diagnoses tab is now available on your clinical sidebar, displaying the patient's diagnosis records and history.
Tip
Once you have enabled the Diagnoses tab, you will be able to add new records directly from the sidebar by clicking on + New Record.
