Create a seamless experience for your patients when filling up a form from your website. With Halaxy, you will be able to embed your forms directly to your website, allowing your patients to complete the necessary forms without the need to navigate away from your website.
Tip
If you are not familiar with HTML code, you may need to ask your website developer about this process.
Insert your form into your website by copying and pasting the provided embed code.

Click Settings > General.

Click Patient forms.

From the forms list, look for any form you would like to embed to your website. Under Actions, click on the
three dots icon and select
Share settings.

In the pop-up, under Embedded Form, click on Copy to copy the whole code and paste it in the body of your website source code.
After pasting the embed code and save the changes to your website, your form will now be displayed and completed directly from your website.
Note
When a patient or professional contact completes an embedded form, their new patient profile and professional contact are automatically labelled Created via Form Link. This label can be removed from any patient profile if needed. However, the label itself cannot be edited in the label settings.
Tip
The completed forms are located within the patient's clinical notes tab.
Important
If an existing patient completes a public patient form, a new profile may be created. You can easily merge the duplicate profile with their existing one.
